ConflictingChartUpdates: Two clinicians edited the same artifact concurrently; both versions shown side-by-side with merge / keep options; merge identity is logged.
DelayedSync: Local cache active with queued-events count and time-behind indicator; for multi-site, mobile clinics, brief network blips.
DisconnectedMonitor: Failure state shown when a critical device stops reporting; no extrapolation from cache > 5 min, auto-pages biomedical engineering.
DowntimeMode: EHR connectivity lost; read-only cached view with new orders queued for sync, visually distinct banner so it cannot be confused with live.
DuplicatePatientDetection: Two records appear to match; merge requires two-clinician verification when records have any conflicting clinical data.
ExpiredSessionDuringSigning: Session lock during a signing event preserves the draft and forces re-authentication; clinical work is never discarded.

The wrong way next to the right way, each grounded in a documented clinical incident, ISMP recommendation, FDA guidance, or accessibility standard.

AIOutputWithoutConfidence: Bare model percentage without version, sources, or human-review requirement; the pattern this exists to prevent (per FDA GMLP guidance).
ColorOnlySeverityEncoding: Severity in hue alone, fails for colorblind users and on monochrome printouts; WCAG 1.4.1 violation.
ConfirmationFatigue: Routine actions wrapped in modal confirmations train clinicians to dismiss the modal that actually matters; reserve confirmations for irreversibles.
CriticalAlertsBehindTabs: P1 alerts collapsed into a tab badge that the 3 a.m. nurse will miss; alerts are persistent UI, not data points.
DarkPatternConsent: Pre-checked research opt-in or buried opt-outs; an IRB and GDPR violation regardless of conversion impact.
DoseEntryWithoutUnits: Numeric dose input without a required unit; the Therac-25 and pediatric chemo error class.
LookAlikeDrugNamesPlain: Plain-text DOPamine vs DOBUTamine in CPOE — ISMP's top medication-error category; mitigated with Tall-Man lettering.

TruncatedIdentifiers: Patient names or MRNs silently cut off — a wrong-patient class of error; two identifiers always visible at full length per TJC NPSG.01.01.01.
VagueSuccessToast: "Saved" tells nothing about what happened to whom; specific subject, specific action, undo when reversible.

Rounds happen on iPads. Cart workstations get gloved hands. Desktop doesn't translate.

CartTouchscreen: Large-target (≥56 px) gloved-hand interaction for cart and OR sterile environments; Code Blue anchored to bottom-right thumb position.
iPadRoundsLayout: Single-column, swipeable patient cards with the primary action in the thumb-reach zone; landscape or portrait, no hover-only states.
